DNA Sequencing Service - General Order Information
	Instructions for Data Iutput
1)	Complete all information listed on the DNA Sequencing Order Form (Order Form Download)
2)	Email (sequencing@base-asia.com, ordering@healthcarehk.com) completed DNA
	Sequencing Order Form and indicate sample collection time
	Data Output
1)	Soft copies of electropherogram and sequencing in text file will be sent through email
	Others
1)	Unless specified in the order form, all samples and primers will be discarded 1 week
	after email of soft copy results
2)	All samples and sequencing data will be kept confidential
